SASH for All or “Support and Services at Home for All,” is a pilot initiative funded by a federal grant. The overall goal to work with residents to provide a broad and flexible spectrum of support and resources within the home to empower participants to achieve goals they have set for themselves. SASH for All is a completely free and voluntary program. The housing-based SASH staff works with individuals and families to assess your health and wellness needs, connect you with resources, and help you meet your goals — goals you decide for yourself.
What Do SASH Participants Receive?
- In-person visits with the staff and a comprehensive Health & Wellness Assessment, updated yearly
- A team-based and person-centered approach to assist you in meeting your self-identified goals
- One-on-one support and regular access to health promotion and prevention-based programs
- A personalized and/or family Healthy Living Plan, based on your self-identified goals
- Access to fun and informative events as part of your local Community Healthy Living Plan — including community meals, educational programs, workshops, exercise classes and more!
- Regular check-ins and support from your SASH for All Coordinator, Wellness Nurse and Peer Support Specialist
- General referrals to help you obtain information on services and resource options available within the community
- An informed team to help in a crisis or special time of need

Who is eligible?
- SASH for All is available to individuals under the age of 65 living in the following affordable housing sites:
- Ledgewood Heights
- Moore Court
