Shelter Plus Care & Transitions to Housing

Shelter Plus Care Program (S+C)/ Coc PSH
The Shelter Plus Care Program provides HUD/CPD rental assistance for hard-to-serve homeless persons with disabilities in connection with supportive services which are funded from sources outside the program.

Shelter Plus Care (S+C) is a program designed to provide housing and supportive services on an individualized basis for chronically homeless persons/families with longstanding disabilities; mental illness, substance abuse, acquired immunodeficiency syndrome; AIDS or related diseases and are living in places not intended for human habitation (e.g., streets, abandoned buildings) or in emergency shelters. The program allows for a variety of housing choices and a range of supportive services. Case management is provided by Morningside Shelter, HCRS and the Brattleboro Area Drop-in Center, only. This program serves approximately 25 participants.

Transitions to Housing
Is a State of Vermont Agency of Human Services funded 2 year transitional housing and supportive services program for qualified homeless families. Applicants/participants must receive intensive weekly case management from an approved community service provider. The majority of participant families in this program are served by the Winston-Prouty’s Family Supported Housing case managers. This program is administered by the BHA and can serve up to approximately 10 families.
